Under general supervision, this position serves on a multi-disciplinary team that provides wraparound services for youth and family in the community setting. This position will be responsible for providing crisis intervention and long-term case management services for at risk youth and their families in the community, while collaborating with community partners (Recreation and Parks, Columbus City Schools, etc). It will provide assessment, screening, care planning, brief therapy sessions, linkage to resources, as well as completing documentation. Additionally, this position will provide education on trauma and resiliency. Performs related duties as required. Evening and weekend hours are required. Preferred:Masters Degree in Social Work, with a Social Work License (LSW) or Masters in Counseling, with a Clinical Counselors License (LPC).
